complete, depending on how much thought you have given to flexible work and
how much detail you provide. To fill out the questionnaire, follow the directions to
the left. Pay Pal will give you access to the web page.

Feedback Report:

The Feedback Report consists of 4 components:

1. Feedback on your current environment and plans for flexible work.

This section summarizes the input you provided and organizes it to prepare you

to focus on what type of flexible work solution is right for you.

2. Brief background on the prevalence of flexibility in your industry.

Examples will be given that match your career type in your current industry.

Companies who illustrate balanced support will be cited.

3. Suggestions for next steps.

We will outline your next five steps to getting ready to implement flexible work in

your career. You may be directed to gather more information about yourself and

your desired company(s). You will also receive information about how to prepare

a proposal for flexible work.

4. Resources, where to get help.

This may include web sites, articles, books, and relevant industry organizations.
